Client population served: 18 & older; acute care or extended clients with a psychiatric illness & diagnoses; clients who are developmentally disabled; geriatric clients; physically challenged along with a psychiatric diagnosis; forensic clients who are mentally ill who is forensic court involved clients. Plans, organizes, conducts & ensures implementation of Activity Therapy program/services (e.g., Leisure Awareness, Community Resources, Arts & Crafts, Dance, Bingo, Music Club and etc.); ensures programming is therapeutic & goal reaching, & conducts individual & group activity therapy sessions; evaluates patients; works with unit treatment (recovery) teams to formulate goals & implement activity therapy interventions identified on treatment plans; prepares & maintains work related documentation (i.e. purchase requests, program inventories, etc.); completes clinical documentation to assure compliance with accrediting/certifying agencies (Medicare/Medicaid & Joint Commission); may provide activity therapy related work direction to assigned volunteers &/or patients involved from hospital &/or community; completes clinical documentation to assure compliance with accrediting/certifying agencies (e.g., Joint Commission, Medicare): accurately completes assessments (e.g., initial comprehensive & annual); completes other clinical documentation (e.g., progress reports which identify & document program statistics & services; prepares (e.g., gathers data, checks for accuracy, distributes, formats) reports &/or documentation as directed by supervisor, completes & submits other program designs &/or documentation as requested by department head &/or supervisor; prepares computer-generated reports as required.
Prepares, completes & maintains all required records; completes activty therapy assessments based on referrals & patient needs, updates patients treatments plans to inlcude activity therapy goals & interventions; writes in progress notes on assigned individuals; recommends and completes requisition forms to order recreation supplies and equipment, completes required forms such as when charting individual behavioral observations; completes request for equipment maintenance & repairs, keeps attendance for general activities; completes incident &/or injury reports when needed.; follows NBH &/or state policies, procedures & safety practices; tilizes Ohio Department of Mental Health & Addiction Services approved office productivity software, approved intra and inter-net resources and information systems in an appropriate work related manner as specified in the NBH Personal Computer Usage Guidelines for Employees. Performs various work related miscellaneous tasks: maintains program equipment & supplies; set up/takes downs needed equipment for various functions; inspects program work area for safe & sanitary conditions; transports & escort clients to community & hospital based programs; pparticipates & attends meetings, conferences/seminars &/or workshop sessions; performs various work related miscellaneous tasks; and completes mandated training; provides clinical supervision of discipline or specialty area for assigned student practicums, internships & volunteers: monitors & evaluates programs related to treatment & discipline Activity Therapy program/services (e.g., Leisure Awareness, Community Resources, Arts & Crafts, Dance, Bingo, Music Club and etc.).
Completion of undergraduate core program in recreation area (e.g., physical education).
-Or completion of associate degree in recreation; 12 mos. Exp. In activity therapy or recreation.
-Or 24 mos. Exp. As Activities, Aide, 44210.
-Or equivalent of Minimum Class Qualifications for Employment noted above.
